次のとおり一般競争入札に付します。

1競争入札に付する事項

6. (6)入札方法
  総合評価落札方式にて入札に付すので、下記3(4)に定める期日までに、提案書等、入札書を提出すること。なお、落札決定に当たっては、入札書に記載された金額に当該金額の10パーセントに相当する額を加算した金額（当該金額に１円未満の端数があるときは、その端数金額を切り捨てるものとする｡)をもって落札価格とするので、入札者は、消費税等に係る課税事業者であるか免税事業者であるかを問わず、見積もった契約金額の110分の100に相当する金額を入札書に記載すること。
2競争参加資格
1. (1)予算決算及び会計令第７０条の規定に該当しない者であること。なお､未成年者､被保佐人又は被補助人であっても､契約締結のために必要な同意を得ている者は､同条中､特別の理由がある場合に該当する。
2. (2)予算決算及び会計令第７１条の規定に該当しない者であること。
3. (3)厚生労働省から指名停止の措置を受けている期間中の者でないこと。
4. (4)令和４・５・６年度厚生労働省競争参加資格（全省庁統一資格）において､厚生労働省大臣官房会計課長から「役務の提供等」で､「A」等級に格付けされ､関東・甲信越地域の競争参加資格を有する者であること。
5. (5)資格審査申請書に虚偽の事実を記載していないと認められる者であること。
6. (6)経営の状況又は信用度が極度に悪化していないと認められる者であること。
7. (7)社会保険等（厚生年金保健､健康保険（全国健康保険協会が管掌するもの）､船員制度､国民年金､労働者災害補償保険及び雇用保険をいう。）に加入し､該当する制度の保険料の滞納がないこと。
8. (8)その他予算決算及び会計令第７３条の規定に基づき､支出負担行為担当官が定める資格を有する者であること。
9. (9)この入札の入札書提出期限の直近１年間において、厚生労働省が所管する法令に違反したことにより送検され、行政処分を受け、又は行政指導（行政機関から公表されたものに限る。）を受けた者にあっては、本件業務の公正な実施又は本件業務に対する国民の信頼の確保に支障を及ぼすおそれがないこと。
10. (10)その他、以下の要件を満たすこと。
  1. ①プライバシーマーク付与認定、ISO/IEC27001認証（国際規格）、JIS Q 27001認証（日本産業規格）のうち、いずれかを取得していること。
  2. ②本業務の作業場所及びデータの保管場所は、日本国内とすること。
  3. ③過去３か年分の財務諸表を提出し、経営状態が健全であることを証明すること。また、当該財務諸表には、公認会計士若しくは監査法人による監査報告書の写し、又は、民間で使用されている「中小企業の会計に関する指針の適用に関するチェックリスト」（日本税理士会連合会作成）若しくは「中小企業の会計に関する基本要領の適用に関するチェックリスト」（日本税理士会連合会作成）を用いて税理士が確認した結果の写しを添付すること。
  4. ④本調達仕様書「１１．（３）応札希望者が閲覧できる資料一覧」を指定期間内に閲覧すること。
  5. ⑤過去に本事業と同等規模以上の類似業務の実績を有していること。また、以下に示すシステムに係る運用・保守を行った実績を過去３年以内に有することが望ましい。単一の実績において以下のすべてを満たす必要はなく、複数の実績の組合せにより満たすことでもよい。
    - 本システムと同程度又はそれ以上の規模の類似システム
    - 個人情報を含むデータを取り扱うシステム
    - 年間延べ12,000人以上の申請者（個人・団体利用者を含む）からのインターネット回線を通じ申請、審査機能を有する情報システムの運用・保守を行った実績を過去３年以内に１年(12ヶ月)以上の実績を有すること。
    - クラウドサービスを用いたＷＥＢサイトシステムの運用保守業務の実績を有していること。
  6. ⑥本業務及び情報セキュリティ管理の履行可能性を証明するため、以下の書類を提出すること。なお、提出された書類において履行可能性を認めることができないと厚生労働省が判断した場合は、入札に参加することはできない。
    - 本調達仕様書に基づいた運用保守業務実施方針書（案）
      なお、ＩＴサービスマネジメントのフレームワーク（ＩＴＩＬ等）を参考に作成すること。
    - 「６．（３）情報セキュリティ管理」に基づいた情報セキュリティ管理計画書（案）
      なお、本業務で取扱う情報等の特性を十分に踏まえて作成したものであること。
  7. ⑦情報システムの調達の公平性を確保するため、応札希望者は、以下に挙げる事業者並びにこの事業者の「財務諸表等の用語、様式及び作成方法に関する規則」（昭和38年大蔵省令第59号）第8条に規定する親会社及び子会社、同一の親会社を持つ会社並びに委託先事業者等の緊密な利害関係を有する事業者でないこと。
    - 「令和3～5年度厚生労働省全体管理組織(PMO）の支援【調達支援等】一式」の受託者
    - 「医薬品等輸入確認情報システム整備に伴う工程管理支援業務一式」の受託者

3. (3)入札者に要求される事項　この一般競争に参加を希望する者は、本公告に示した業務ができることを証明する書類を指定する期日までに提出しなければならない。また、入札に参加を希望する者は、上記証明書類とあわせて暴力団等に該当しない旨の誓約書を提出しなければならない。入札者は、支出負担行為担当官から当該書類に関し説明を求められた場合は、それに応じなければならない。
4. (4)入札の無効　本公告に示した競争参加資格のない者の提出した入札書、入札者に求められる義務を履行しなかった者の提出した入札書、その他入札の条件に違反した者の提出した入札書は無効とする。また、入札に参加した者が、(3)の誓約書を提出せず、又は虚偽の誓約をし、若しくは誓約書に反することとなったときは、当該者の入札を無効とするものとする。

(6)落札者の決定方法　本公告に示した業務を履行できると支出負担行為担当官が判断した入札者であって、予算決算及び会計令第79条の規定に基づいて作成された予定価格の制限の範囲内であり、提案書作成要領・評価基準書で指定する技術等の要件のうち、必須とした項目について基準を全て満たしている提案をした入札者での中から、提案書作成要領・評価基準書で定める総合評価の方法をもって落札者を決定する。ただし、落札者となるべき者の入札価格によっては、その者により当該契約の内容に適合した履行がなされないおそれがあると認められるとき、又はその者と契約を締結することが公正な取引の秩序を乱すこととなるおそれがあって著しく不適当であると認められるときは、予定価格の制限の範囲内の価格をもって入札した他の者のうち、総合評価の最も高い者を落札者とすることがある。

6. (6)Qualifications for participating in the tendering procedures:
  Suppliers eligible for participating in the proposed tender are those who shall: [1] Not come under Article 70 of the Cabinet Order concerning the Budget, Auditing and Accounting. Furthermore, minors, Person under Conservatorship or Person under Assistance that obtained the consent necessary for concluding a contract may be applicable under cases of special reasons within the said clause [2] Not come under Article 71 of the Cabinet Order concerning the Budget, Auditing and Accounting [3] Are not being suspended from transactions by request of the officials in charge of contract [4] Have Grade “A” on “offer of services etc.” in the Kanto- Koshinetsu Area in terms of qualification for participating in tenders by Ministry of Health, Labour and Welfare (Single qualification for every ministry and agency) in the fiscal years 2022,2023 and 2024 [5] Prove to have no false statement in tendering documents [6] Prove neither the business condition nor credibility is deteriorating [7] Meet the qualification requirements which the Obligating Officer may specify in accordance with Article 73 of the Cabinet Order [8] Acquire any of the following: privacy mark certification, ISO/IEC27001 certification (international standard), or JIS Q 27001 certification (Japanese industrial standard). [9] The work place for this work and the data storage place shall be in Japan. [10] Submit financial statements for the past three years to prove that the business is sound. In addition, the financial statements include a copy of an audit report by a certified public accountant or an audit firm, or a “checklist for the application of guidelines for accounting for small and medium-sized enterprises” (prepared by the Japan Federation of Tax Accountants' Associations) or Attach a copy of the result of checking by a tax accountant using the "Checklist for the application of basic guidelines for accounting for small and medium-sized enterprises" (prepared by the Japan Federation of Tax Accountants' Associations). [11] “11.(3) List of materials that can be viewed by prospective bidders” listed in the procurement specification of this services shall be viewed within the specified period. [12] Applicants must have a track record of similar work of equal or greater scale to this project in the past. And it is desirable that the applicant has a track record of operation and maintenance related to the following systems within the past three years. It is not necessary for a single achievement to satisfy all of the following, and a combination of multiple achievements may be used.
  - Similar systems of the same or larger scale as this system.
  - Systems that handle data containing personal information.
  - One year (12 months) within the past three years of operating and maintaining an information system that has application and examination functions from more than 12,000 applicants (including individual and group users) annually or above.
  - Have a track record of operating and maintaining a website system using a cloud service.
  [13] Submit the following documents to prove the feasibility of this work and information security management. In addition, if the Ministry of Health, Labor and Welfare determines that the submitted documents cannot confirm the possibility of fulfillment, the applicant will not be able to participate in the tender.
  - Operating and maintenance work implementation policy based on the procurement specification of this services (draft)
  It should be created with reference to the IT service management framework (ITIL, etc.).
  - Information security management plan (draft) based on “6. (3) Information security management” listed in the procurement specification of this services
  In addition, it must be created with sufficient consideration of the characteristics of the information, etc. handled in this service.
  [14] In order to ensure fairness in the procurement of information systems, potential bidders should comply with the following business operators and the "Regulations Concerning Terminology, Forms and Preparation Methods for Financial Statements" (Ministry of Finance Ordinance No. 59 of 1963) ) It is not a parent company and subsidiary company prescribed in Article 8, a company with the same parent company, or a business operator with a close interest such as a subcontractor business operator.
