6. (6)入札方法
  入札金額は総価を記入すること。落札者の決定は、総合評価落札方式をもって行うので、下記3(2)に定める期日までに、技術提案書類等を提出すること。なお、落札決定に当たっては、入札書に記載された金額に当該金額の10パーセントに相当する額を加算した金額（当該金額に１円未満の端数があるときは、その端数金額を切り捨てるものとする。）をもって落札価格とするので、入札者は、消費税に係る課税事業者であるか免税事業者であるかを問わず、見積もった契約金額の110分の100に相当する金額を入札書に記載すること。
2競争参加資格
1. (1)予算決算及び会計令第70条の規定に該当しない者であること。なお、未成年者、被保佐人又は被補助人であって、契約締結のために必要な同意を得ている者は、同条中、特別の理由がある場合に該当する。
2. (2)予算決算及び会計令第71条の規定に該当しない者であること。
3. (3)令和４・５・６年度厚生労働省競争参加資格（全省庁統一資格）「役務の提供等」で｢Ａ｣又は｢Ｂ｣等級に格付けされ、関東・甲信越地域の競争参加資格を有する者であること。
4. (4)資格審査申請書又は添付書類に虚偽の事実を記載していないと認められる者であること。
5. (5)経営の状況又は信用度が極度に悪化していないと認められる者であること。
6. (6)以下に掲げる制度が適用される者にあっては、この入札の入札書提出期限の直近２年間（オおよびカについては２保険年度）の保険料について滞納がないこと。

  イ　健康保険（全国健康保険協会が管掌するもの）
  ウ　船員保険
  エ　国民年金
  オ　労働者災害補償保険

7. (7)以下のいずれかを取得していること。また、取得していることを証明するための書類（様式任意）を提出すること。

  イ　ISMS(JISQ27001)(ISO/IEC27001)
8. (8)厚生労働省から指名停止の措置を受けている期間中ではないこと。
9. (9)過去に同等規模以上の類似業務の実績を有していること。
10. (10)その他予決令第73条の規定に基づき、支出負担行為担当官が定める資格を有する者であること。
11. (11)過去１年以内に厚生労働省所管法令違反により行政処分を受けていないこと。
  ただし、労働基準関係法令違反（※）により労働基準監督機関から使用停止等命令を受けたが、是正措置を行い「使用停止等命令解除通知書」を受理している場合には、この限りではない。
  ※労働基準関係法令については以下のとおり。　
  労働基準法、労働安全衛生法、最低賃金法、賃金の支払の確保等に関する法律、家内労働法、作業環境測定法、じん肺法、炭鉱災害による一酸化炭素中毒症に関する特別措置法
12. (12)過去１年以内に厚生労働省所管法令違反により送検され、この事実を公表されていないこと。
13. (13)過去１年以内に厚生労働省所管法令に基づく公表制度により、又は違法な長時間労働や過労死等が複数の事業場で認められた企業として、平成29年1月20日付け基発0120第1号厚生労働省労働基準局長通達「違法な長時間労働や過労死等が複数の事業場で認められた企業の経営トップに対する都道府県労働局長等による指導の実施及び企業名の公表について」記３、平成31年１月25日付け基発0125第１号「裁量労働制の不適正な運用が複数の事業場で認められた企業の経営トップに対する都道府県労働局長による指導の実施及び企業名の公表について」記の３に基づく企業名の公表をされていないこと。

  原則、電子メールで提出すること。
  未着の場合、その責任は参加者に属するものとし、期限内の提出がなかったものとみなす

  令和５年１月24日（火）～令和５年３月15日（水）
5. (5)入札説明書の交付方法
  郵送又は電子メールにて対応する。
  郵送での交付を希望する場合は、返信用封筒と担当者の連絡先が分かる物（名刺等）を同封のうえ上記（１）の宛先まで送付すること。
  電子メールでの交付を希望する場合は、同様に上記（１）まで電子メールにて交付希望の旨連絡すること。
  交付の申込みは、期限までに到着するように送付しなければならない。未着の場合、その責任は参加者に属するものとし、期限内の送付がなかったものとみなす。

  オンラインにて開催を予定。
  参加を希望する場合、開催日の３日前までに上記３(1)の宛先にその旨をメールで伝えること。なお、３日前までに申し込みのなかった場合は不参加とみなす。

4. (4)入札書の提出方法
  原則、郵送（書留郵便に限る）又は持参で提出すること。
  （今般の事情に鑑み、不落札の場合を想定し、事前に入札書を複数提出することを可能とする。再入札用の入札書には、封筒に｢再入札　○回目｣と記載すること。）
  未着の場合、その責任は参加者に属するものとし、期限内の提出がなかったものとみなす。

  この入札に参加を希望する者は、上記２の競争参加資格を有することを証明する書類を令和５年３月16日（木）16時00分までに郵送で上記４（１）の宛先まで提出しなければならない。
  また、入札に参加を希望する者は、上記証明書類と合わせて、支出負担行為担当官が指定する暴力団等に該当しない旨を記した誓約書を提出しなければならない。
  入札者は、支出負担行為担当官から当該書類に関し説明を求められた場合は、これに応じなければならない。
4. (4)入札の無効
  本公告に示した競争参加資格のない者の提出した入札書、入札者に求められる義務を履行しなかった者の提出した入札書、その他入札の条件に違反した者の提出した入札書は無効とする。また、(3)の誓約書を提出せず、又は虚偽の誓約をし、若しくは誓約書に反することとなった時は、当該者の入札を無効とする。

6. (6)落札者の決定方法
  予算決算及び会計令第79条の規定に基づいて作成された予定価格の制限の範囲内であり、入札説明書等で指定する技術等の要件のうち必須とされた項目の最低限の要求要件をすべて満たしている提案をした入札者の中から、入札説明書等で定める総合評価落札方式の方法をもって落札者の決定をする。
  ただし、落札者となるべき者の入札価格によっては、その者により当該契約の内容に適合した履行がなされないおそれがあると認められるとき、又はその者と契約を締結することが公正な取引の秩序を乱すこととなるおそれがあって著しく不適当であると認められるときは、予定価格の制限の範囲内の価格をもって入札した他の者のうち、総合評価点の最も高い者を落札者とすることがある。

9. (9)契約締結日までに国の予算（暫定予算を含む。）が成立していない場合は、契約締結日は予算が成立した日以降とする。また暫定予算となった場合は、契約内容等について変更が生じる可能性や本事業に係る予算の決定状況によって仕様の内容について変更が生じる可能性があるので、その際は双方で別途協議する。

6. (6)Qualifications for participating in the tendering procedures: Suppliers eligible for participating in the proposed tender are those who shall:
  [1] Not come under Article 70 of the Cabinet Order concerning the Budget, Auditing and Accounting. Furthermore, minors, Person under Conservatorship or Person under Assistance that obtained the consent necessary for concluding a contract may be applicable under cases of special reasons within the said clause.
  [2] Not come under Article 71 of the Cabinet Order concerning the Budget, Auditing and Accounting.
  [3] Have Grade ｢A｣ or ｢B｣ on " offer of services etc. " in terms of qualification for participating in tenders by Ministry of Health, Labour and Welfare(Single qualification for every ministry and agency)in the fiscal years,2022, 2023 and 2024.
  [4] Prove to have no false statement in tendering documents.
  [5] Prove neither the business condition nor credibility is deteriorating.
  [6] For bidders subject to the following insurance system of each item , and it is not in arrears either of insurance premiums (2 insurance year about (ⅴ) and (ⅵ)) for the last two years of the bid submission deadline of this bid：
  (ⅰ)　Welfare annuity insurance
  (ⅱ)　Health insurance (the thing which association of national health in surance has jurisdiction over)
  (ⅲ)　Seamen's insurance
  (ⅳ)　National pension
  (ⅴ)　Worker's compensation insurance

  [7]It is necessary to get the following　authentication.
  (ⅰ) Privacy Mark (JISQ15001)
  (ⅱ) ISMS (ISO/IEC27001 or JISQ27001)
  [8] Do not go out of Ministry of Health Labour and Welfare about duties during a period taking a nomination stop.
  [9] Have a performance of similar work of equivalent or larger scale in the past.
  [10] Meet the qualification requirements which the Obligating Officer may specify in accordance with Article 73 of the Cabinet Order concerning the Budget, Auditing and Accounting.
  [11] Not received any administrative sanctions for violation of the Laws and Regulations under the jurisdiction Ministry of Labour, Health and Welfare within 1 year. However, this shall not apply in the case where the supplier took corrective actions and received “Notification of withdrawing for the order of suspension of use” after received an order of suspension of use by Labour Standard Inspection Administration, due to violation of the Labour Standard related laws and regulations(*).
  (*) Labour Standards Law, Industrial Safety and Health Act, Minimum Wage Act, Act on Securing the Payment of Wages, Industrial Homework Act, Working Environment Measurement Act, Pneumoconiosis Act, Act on Special Measures concerning Carbon Monoxide Poisoning Caused by Coal-Mine Accident
  [12] Not be sent the documents to the public prosecutor’s office due to violation of the Laws and Regulations under the jurisdiction Ministry of Labour, Health and Welfare and be made this fact public.
  [13] Be accompanied by by the publication system based on Ministry of Labour, Health and Welfare jurisdiction laws and ordinances or illegal long working hours or death from overwork as an approved company in plural business ground on January 20, 2017 within the past one year and do not be announced the company name based on 3 of the note about Director from basis 0120 first Ministry of Labour, Health and Welfare standards notification "enforcement of the instruction by Director metropolis and districts labor for the top management of the company where illegal long working hours or death from overwork are plural business ground and were admitted and from basis 0125 first "enforcement of the instruction by Director metropolis and districts labor for the top management of the company where the non-appropriate operation of the discretionary working system is plural business ground and was admitted about the publication of the company name on note 3, January 25, 2019 dated and the publication of the company name".
