      入札金額は総価で行う。
      なお、落札決定に当たっては、入札書に記載された金額に当該金額の10パーセントに相当する額を加算した金額（当該金額に１円未満の端数があるときは、その端数金額を切り捨てるものとする。）をもって落札価格とするので、入札者は、消費税に係る課税事業者であるか免税事業者であるかを問わず、見積もった契約金額の110分の100に相当する金額を入札書に記載すること。
    6. (6)電子調達システムの利用
      本案件は、電子調達システムで行う。
      なお、電子調達システムによりがたい者は、書面により申し出た場合に限り、紙入札方式に代えることができる。紙入札方式の場合については、郵送にて厚生労働省競争参加資格（全省庁統一資格）及び入札書の提出を行う。
  2. 2競争参加資格
    1. (1)予算決算及び会計令第70条の規定に該当しない者であること。 なお、未成年者、被保佐人又は被補助人であって契約締結のために必要な同意を得ている者は、同条中、特別の理由がある場合に該当する。
    2. (2)予算決算及び会計令第71条の規定に該当しない者であること。
    3. (3)令和４・５・６年度厚生労働省競争参加資格（全省庁統一資格） において、「物品の製造」のうち営業品目が「フォーム印刷」又は「その他印刷類」を有しており、「Ｂ」、「Ｃ」又は「Ｄ」等級のいずれかの等級に格付けされている者であること。
    4. (4)資格審査申請書又は添付書類に虚偽の事実を記載していないと認められる者であること。
    5. (5)経営の状況又は信用度が極度に悪化していないと認められる者であること。
    6. (6)次の各号に掲げる制度が適用される者にあっては、この入札の入札書提出期限の直近２年間（オ及びカについては２保険年度）の保険料について滞納がないこと。
      ア厚生年金保険　イ健康保険（全国健康保険協会が管掌するもの）　ウ船員保険　エ国民年金　オ労働者災害補償保険　カ雇用保険
    7. (7)当該物品又はこれと類似の物品を相当量製造納入した実績を有する者であること。
    8. (8)必要な用紙を確実に入手できると認められる者であること。
    9. (9)本調達において当方で指定した試作品を事前に提出し、当方が実施する試作品試験に合格した者であること。
    10. (10)厚生労働省から指名停止を受けている期間中の者でないこと。
    11. (11)その他予算決算及び会計令第73条の規定に基づき、支出負担行為担当官が定める資格を有する者であること。
      ア　過去１年以内に厚生労働省所管法令違反により行政処分を受けていないこと。ただし、労働基準関係法令違反（※）により労働基準監督機関から使用停止等命令を受けたが、是正措置を行い「使用停止等命令解除通知書」を受理している場合には、この限りではない。
      イ　過去１年以内に厚生労働省所管法令違反により送検され、この事実を公表されていないこと。
      ウ　過去１年以内に厚生労働省所管法令に基づく公表制度により、又は違法な長時間労働や過労死等が複数の事業場で認められた企業として、平成29年１月20日付け基発0120第１号 厚生労働省労働基準局長通達「違法な長時間労働や過労死等が複数の事業場で認められた企業の経営トップに対する都道府県労働局長等による指導の実施及び企業名の公表について」記３ に基づく企業名の公表をされていないこと。

      この一般競争に参加する者は、製造物品の品質、特質及び条件に適合する試作品を、上記３（４）に示した期限までに提出場所へ提出しなければならない。入札者の試作品は当省において審査し、採用し得ると判断された場合の入札書のみを落札決定の対象とする。
      なお、入札者は、支出負担行為担当官が指定する暴力団等に該当しない旨を記した誓約書を提出しなければならない。
      また、入札者は、開札日の前日までの間において、支出負担行為担当官から当該書類等に関し説明を求められた場合は、それに応じなければならない。
    4. (4)入札時留意事項
      ア　入札者から提出される入札・契約関係書類については、事業者としての決定であることを要する。
      イ　入札・契約関係書類に虚偽記載等の不正が発覚した場合は、契約解除や違約金を徴取することがある。
    5. (5)入札の無効
      上記２に示した参加競争資格のない者の提出した入札書、上記（３）に示した入札者に要求される事項を履行しなかった者の提出した入札書、その他入札の条件に違反した者の提出した入札書は無効とする。
    6. (6)契約書作成の要否
      落札者との契約締結にあたり、会計法第２９条の８の規定により契約書の作成を要する。
    7. (7)落札者の決定方法
      本公告に示した業務を履行できると支出負担行為担当官が判断した入札者であって、予算決算及び会計令第79条の規定に基づいて作成された予定価格の制限の範囲内で最低価格をもって有効な入札を行った者を落札者とする。
