Questionnaire (for those who are unable to use “Visit Japan Web”)

Those who are unable to use “Visit Japan Web” must confirm their required quarantine documents prior to boarding the aircraft in the country of departure, and at the point of arrival in Japan *, in addition to answering the questionnaire.

*Please note that the confirmation of your documents will take time. If you have a smartphone or PC, please pre-register the required documents on “Visit Japan Web”.

(1) If you are unable to use “Visit Japan Web” and will be entering Japan at the following airports, please pre-register the questionnaire online before entering Japan.

New Chitose Airport, Sendai International Airport, Narita International Airport, Haneda Airport, Chubu Centrair International Airport, Kansai International Airport, Hiroshima Airport, Takamatsu Airport, Fukuoka Airport, Naha Airport

For people cannot use "Visit Japan Web" system

Please pre-register questionnaire via web to issue the QR code, and saved it as a screenshot or printed out to present at quarantine.

(2) If you are entering Japan at any other airport than the ones listed above, please complete and print the following questionnaire before entering Japan and submit it to the quarantine station at the airport of arrival.
