次のとおり一般競争入札（最低価格落札方式）に付します。

１　調達内容

 (6)  入札方法　

落札者の決定は最低価格落札方式により行うものである。

〔１〕入札者は、調達件名の本体価格のほか、業務の履行に要する一切の諸経費を含めた契約金額を見積もるものとする。

〔２〕落札決定に当たっては、入札書に記載された金額に当該金額の10パーセントに相当する額を加算した金額（当該金額に1円未満の端数があるときは、その端数金額を切り捨てるものとする。）をもって落札価格とするので、入札者は消費税に係る課税事業者であるか免税事業者であるかを問わず、見積もった契約金額の110分の100に相当する金額を記載した入札書を提出しなければならない。

 

２　競争参加資格

 (1)　予算決算及び会計令第70条の規定に該当しない者であること。

なお、未成年者、被保佐人又は被補助人であって、契約締結のために必要な同意を得ている者は、同条中、特別の理由がある場合に該当する。

 (2)　予算決算及び会計令第71条の規定に該当しない者であること。

 (3)　 令和01・02・03年度（平成31・32・33年度）厚生労働省競争参加資格（全省庁統一資格）において、｢役務の提供等｣で「Ａ」又は「Ｂ」のいずれかの等級に格付けされている者であること。

(4)　 次の各号に掲げる制度が適用される者にあっては、この入札の入札書提出期限の直近2年間（オ及びカについては2保険年度）の保険料について滞納がないこと。

ア 厚生年金保険

イ 健康保険（全国健康保険協会が管掌するもの）

ウ 船員保険

エ 国民年金

オ 労働者災害補償保険

カ 雇用保険

(5)  資格審査申請書又は添付書類に虚偽の事実を記載していないと認められる者であること。

 (6)  経営の状況又は信用度が極度に悪化していないと認められる者であること。

(7)　厚生労働省から指名停止の措置を受けている期間中の者でないこと。

(8)  その他予算決算及び会計令第73条の規定に基づき、支出負担行為担当官が定める資格を有する者であること。

　　　原則、立会方式での開札は、実施しない。

(4)　 入札書の提出方法

　　  本案件は、紙により厚生労働省競争参加資格（全省庁統一資格）及び入札書の提出並びに開札を行う。電子調達システムによる入札は認めない。

また、入札書は郵送（書留郵便に限る）とし、上記（１）あて入札書の提出期限までに到着するように送付しなければならない。未着の場合、その責任は参加者に属するものとする。

なお、電報、ＦＡＸ、電子メール等その他の方法による提出は認めない。

(2)    入札説明書の交付方法

　　　本公告開始日から令和２年７月28日（火）までの間、上記（１）宛に交付の申込みを行った者に対し、上記（１）より入札説明書を交付する。

(3)   入札説明会の日時及び場所

　　　入札説明会は、実施しない。

 (4)    履行可能性審査書類の提出期限

令和２年７月29日　12時00分

(5) 　 履行可能性審査書類の提出方法

上記（１）あてに郵送(書留郵便に限る。) で提出とするが、上記（１）あてに履行可能性審査書類の提出期限までに到着するように送付しなければならない。未着の場合、その責任は参加者に属するものとし、期限内の提出がなかったものとみなす。　

なお、普通郵便、電報、ＦＡＸ、電子メール等その他の方法による提出は認めない。

 

５　その他

(1)　 契約手続において使用する言語及び通貨　

日本語及び日本国通貨に限る。

 (2)　 入札保証金及び契約保証金　免除

 (3)　 入札者に求められる義務

この入札に参加を希望する者は、入札説明書に示す競争参加資格確認書類を令和２年７月29日12時00分までに提出しなければならない。また、入札者は、支出負担行為担当官から当該書類に関し説明を求められた場合は、これに応じなければならない。

また、履行可能性審査に係る書類として、［１］実施計画書(案)、［２］運用保守業務実施方針書（案）、［３］セキュリティ管理計画書(案)を令和２年７月29日12時00分までに提出しなければならない。

 (4)　 入札の無効

本公告に示した競争参加資格のない者の提出した入札書、入札者に求められる義務を履行しなかった者の提出した入札書、その他入札の条件に違反した者の提出した入札書は無効とする。また、(3)の競争参加資格確認書類のうち、誓約書を提出せず、又は虚偽の誓約をし、若しくは誓約書に反することとなった場合は、当該者の入札を無効とする。

 (6)　落札者の決定方法　

　　　予算決算及び会計令第79条の規定に基づいて作成された予定価格の制限の範囲内で、最低価格をもって有効な入札を行った者を落札者とする。ただし、落札者となるべき者の入札価格によっては、その者により当該契約の内容に適合した履行がなされないおそれがあると認められるとき、又はその者と契約を締結することが公正な取引の秩序を乱すこととなるおそれがあって著しく不適当であると認められるときは、予定価格の制限の範囲内の価格をもって入札した他の者のうち、最低の価格をもって入札した者を落札者とすることがある。

６ Summary

(5) Fulfillment place: The place specified by the Official in charge of disbursement of the procuring entity．

(6) Qualifications for participating in the tendering procedures : suppliers eligible for participating in the proposed tender are those who shall:

［1］ Not come under Article 70 of the Cabinet Order concerning the Budget, Auditing and Accounting. Furthermore, minors, Persons under Conservatorship or Persons under Assistance who have obtained the consent necessary for concluding a contract may be applicable under cases of special reasons within the said clause.

［2］ Not come under Article 71 of the Cabinet Order concerning the Budget, Auditing and Accounting.

［3］ Have Grade “A”or “B”on “Offer of services , etc”in terms of qualification for participating in tenders by Ministry of Health, Labour and Welfare (Single qualification for every ministry and agency) in the fiscal years of 2019, 2020 and 2021.

［4］ For bidders subject to the following insurance system of each item , and it is not in arrears either of insurance premiums (2 insurance year about (�D) and (�E)) for the last two years of the bid submission deadline of this bid：

　  　(�A)　Health insurance (the thing which association of national health in surance has jurisdiction over)

　  　(�B)　Seamen's insurance

　  　(�C)　National pension

　  　(�D)　Worker's compensation insurance

　  　(�E)　Unemployment insurance

[5]  Prove to have no false statement in tendering documents.

[6]  Prove neither the business condition nor credibility is deteriorating.

      ［7］Not in duties during a period taking a nomination stop from Ministry of Health, Labour　and Welfare.

[8] Not received any administrative sanctions for violation of the Laws and Regulations under the jurisdiction Ministry of Labour, Health and Welfare within 1 year. However, this shall not apply in the case where the supplier took corrective actions and received “Notification of withdrawing for the order of suspension of use” after received an order of suspension of use by Labour Standard Inspection Administration, due to violation of the Labour Standard related laws and regulations(*).

[9] Not be sent the documents to the public prosecutor’s office due to violation of the Laws and Regulations under the jurisdiction Ministry of Labour, Health and Welfare and be made this fact public.

Obligating Officer may specify in accordance with

Article 73 of the Cabinet Order.
