次のとおり一般競争入札に付します。

１　調達内容

（１）品目分類番号　71、27

　　　　労働保険適用徴収システムに係る機能改修業務（滞納整理関連機能の改善等に係る改修）

　　　入札説明書及び仕様書による。

（６）入札方法

　　　落札者の決定は最低価格落札方式により行うものである。

　　　�@入札者は、調達件名の本体価格のほか、業務の履行に要する一切の諸経費を含め

　　　　た契約金額を見積もるものとする。

　　　�A落札決定に当たっては、入札書に記載された金額に当該金額の8パーセントに相

　　　　当する額を加算した金額（当該金額に1円未満の端数があるときは、その端数金

　　　　額を切り捨てるものとする。）をもって落札価格とするので、入札者は消費税に係

　　　　る課税事業者であるか免税事業者であるかを問わず、見積もった契約金額の108

　　　　分の100に相当する金額を記載した入札書を提出しなければならない。

 

２　競争参加資格

（１） 予算決算及び会計令第70条の規定に該当しない者であること。なお、未成年者、被保佐人又は被補助人であっても、契約締結のために必要な同意を得ている者は、同条中、特別な理由がある場合に該当する。

（２） 予算決算及び会計令第71条の規定に該当しない者であること。

（３） 平成25・26・27年度厚生労働省競争参加資格（全省庁統一資格）において、「役務の提供等」のうち営業品目が「情報処理」又は「ソフトウェア開発」で「A」、「B」又は「C」の等級に格付けされている者であること。

（４） 資格審査申請書又は添付書類に虚偽の事実を記載していないと認められる者であること。

（５） 経営の状況又は信用度が極度に悪化していないと認められる者であること。

（６） 次の各号に掲げる制度が適用される者にあっては、この入札の入札書提出期限の直近2年間（オ及びカについては2保険年度）の保険料について滞納がないこと。

ア 厚生年金保険

イ 健康保険（全国健康保険協会が管掌するもの）

ウ 船員保険

エ 国民年金

オ 労働者災害補償保険

カ 雇用保険

（７） 厚生労働省から指名停止を受けている期間でないこと。

（８） 徴収システムと同等の開発・実行環境（プログラミング言語及びOS、DBMS等のソフトウェア）であるシステムの改修業務についての実績を有すること。

（９）徴収システムと同等規模システムの追加機能等の改修業務についての実績を有すること。

（10） 情報システムの設計・開発に係る品質管理体制について「ISO9001:2008」、組織としての能力成熟度について「CMMIレベル3以上」のうち、いずれかの認証を受けていること。

（11） 「プライバシーマーク付与認定」、「ISO/IEC27001認証（国際標準）」、「JIS Q 27001認証（日本工業標準）」のうち、いずれかの認証を受けていること。なお、「ISO/IEC27001認証（国際標準）」、「JIS Q 27001認証（日本工業標準）」については、本業務を実施する組織において認証を取得していること。

入札に参加を希望する者は、支出負担行為担当官が指定する暴力団等に該当しない旨を記した誓約書を提出しなければならない。

入札者は、支出負担行為担当官から当該書類に関し説明を求められた場合は、これに応じなければならない。

（４）入札の無効

上記２に示した競争参加資格を有しない者のした入札及び入札に関する条項に違反した入札は無効とする。

また、（３）の誓約書を提出せず、又は虚偽の誓約をし、若しくは誓約書に反することとなったときは、当該者の入札を無効とする。

本公告に示した業務を履行できると支出負担行為担当官が判断した入札者であって、予算決算及び会計令第79条の規定に基づいて作成された予算価格の制限の範囲内で最低価格をもって有効な入札を行った者を落札者とする。

５　Summary

（６）     Qualifications for participating in the tendering procedures : Supplier eligible for participating in the proposed tender are those who shall :

[1]　Not come under Article 70 of the Cabinet Order concerning the Budget, Auditing and Accounting. Furthermore, minors, Persons under Conservatorship or persons under Assistance who have obtained the consent necessary for concluding a contract may be applicable under cases of special reasons within the said clause.

[2]　Not come under Article 71 of the Cabinet Order concerning the Budget, Auditing and Accounting.

[3]　Have Grade "A","B", or "C"in the public tender qualification category of" Information processing" or "Software development" among "Offer of services,etc" by the Ministry of Health, Labour and Welfare (The common qualification for all ministries and agencies) in the fiscal years of 2013, 2014 and 2015.

[4]  Prove to have no false statement in tendering documents.

[5]  Prove neither the business condition nor credibility is deteriorating.

[6]　There not being nonpayment about the premium for most recent two years in a deadline for bid book presentation of this bid about the person whom a system to advocate next is applied to.

[7]  Do not be a period receiving a nomination stop from Ministry of Health, Labour and Welfare.

[8]　Have the results about the repair duties of the system which is development, the practice environment where is equal to an application collection system (software such as a programming language and the OS, the DBMS).

[9]　 Have the results about repai              r duties such as the additional functions of the scale system equal to an application collection system.

[10]　 Receiving either certification among "higher than CMMI level 3" about "ISO9001:2008", an ability maturity degree as the organization about the quality control system affecting a design and development of the information system.

[11]　 Receiving either certification among "privacy mark grant authorization", "the ISO/IEC27001 certification" (international standard), "27001 JIS Q certification" (Japanese industrial standard). In addition, acquiring the
